Step 5:

Ask all applicants to complete a self-declaration form

The self-declaration process provides an opportunity to highlight any potential difficulties/concerns at an early point in the recruitment process and gives an applicant the opportunity to supply information voluntarily.

A discussion about the circumstances, timescales etc. of events detailed in a Self-Declaration Form will help organisations consider the relevance of the information in relation to the position applied for.

Organisations may find an interview process matrix (checklist) is a helpful tool for processing the information provided in a Self-Declaration Form and, where appropriate, during an interview.

It is very important that individuals who are involved in the recruitment process understand that completed Self-Declaration Forms contain personal and sensitive information which must be managed by the organisation in accordance with Data Protection principles.

See toolkit templates for examples of a Self Declaration form and guidance notes to the Self-Declaration Form

As with all the steps detailed in Steps to Recruitment (except for Disclosure checks ), the Act does not legally require organisations to ask individual applicants to complete a self-declaration form.
