Disclosure Application FormAll disclosures are applied for on the same Disclosure Application Form. If you are registered with CRBS, copies can be requested direct. Multiple Cover SheetMultiple Cover Sheets are available from CRBS to all organisations as an aid to minimise paperwork. The utilisation of the Multiple Cover Sheet is also more efficient and cost effective. It enables organisations to send in multiple Disclosure Applications with the details of the applicants listed on the Multiple Cover Sheet. The Signatory will complete the Multiple Cover Sheet with the same information that they are asked for on a single Blue Cover Sheet. If your organisation would like to start using the Multiple Cover Sheet please call our helpline on 01786 849777 and a member of staff will e-mail it to you.
CostsThe CRBS is the only source of free disclosures for volunteers in the voluntary sector in Scotland. Voluntary organisations will also be able to request disclosures for paid staff via the CRBS, but there will be a charge of £20 per disclosure (as of the 1st August 2009 this will rise to £23)
