If your organisation is enrolled with CRBS and wishes to access disclosure records on behalf of other not for profit voluntary sector organisations you must complete and return this form for approval by CRBS before starting the process.

This form must be completed by each of the secondary organisations that an intermediary body wishes to represent. CRBS will not process disclosure applications submitted for a secondary organisation until a contract form has been ccompleted and approved by CRBS. There are two options to choose from:

Option 1: This form should be completed when the disclosure record is to be forwarded to the secondary organisation.

Option 2: This form should be completed when the disclosure record is to be retained by the intermediary organisation.
