Intermediary Paperwork

If your organisation is enrolled with CRBS and wishes to access disclosure certificates on behalf of other organisations you must complete and return this form for approval by CRBS before starting the process.

This information has been designed to help you complete the Intermediary Body Contract Form correctly and to ensure that you manage the process in line with the requirements of the Code of Practice.

This form must be completed by each of the secondary organisations that an intermediary body wishes to represent. CRBS will not process disclosure applications submitted for a secondary organisation until a contract form has been ccompleted and approved by CRBS. There are two options to choose from:

Option 1: This form should be completed when the disclosure certificate is to be forwarded to the secondary organisation.

Option 2: This form should be completed when the disclosure certificate is to be retained by the intermediary organisation.
