Tips for Completing Application Forms


Application to join PVG Scheme

General Points

There are mandatiry fields on the form that must be completed. These fields are highlighted in yellow. Failure to complete these sections will result in your application being delayed.

Ensure the form completed in black or dark blue ink

A coversheet must be completed and submitted with PVG applications

Organisations should detail a job/volunteer title that details why the position/role is regulated work. For example, detailing "Volunteer" is not sufficient information for CRBS to determine that the role is regulated work. Examples of job/volunteer titles that are useful are: Childrens Activities volunteer, Peer Support worker, volunteer befriender, volunteer childrens playworker and Care worker.

All PVG applications should be returned to: CRBS, Jubilee House, Forthside Way, Stirling, FK8 1QZ

A1 - scheme record should always be selected

A3 - organisations cannot apply for an online account if enrolled with CRBS

B23  - If an individual has a UK National Insurance Number, B23 should read "yes" and detailed in B24

B25 - If an individual has a passport, B25 should be "yes" and detailed in sections B26/B27

B28 - If an individual has a driving licence , B28 should read "Yes" and detailed in sections B29/B30

B31 - If an individual has a National Identity vard, B31 should read "Yes" and detailed in sections B32/B33/B34

B36 - If an individual is a member of the PVG Scheme or has ever been a member of the PVG Scheme, B36 should be completed and detailed in section B37.

B38 - If an individual is a member of the ISA or has ever been a member of the ISA, section B38 should be completed and detailed in section B38.

B83 - This section refers to the applicant not the organisation. If the subject of the application is a member of a Regulatory Body e.g if someone is applying to become a Sunday School Teacher with your oganistion and they are also registered with the General Teaching Council for Scotland, they should insert the appropriate Regulatory Body code.

E1 - CRBS can only process PVG applications that relate to new positions within organisations. E1 should always be ticked as "No"

E3 - Should detail the name of the organisation where the individual will carry out the regulated work. Organisatons acting as an Intermediary Body should not detail their organisation name in this section

E5 - Organisations should detail a job/volunteer title that details why the position/role is regulated work. E11 - E15 - These sections should only be completed by CRBS staff.

F1-F2 - This section should be signed by CRBS staff only
